Our office is a combination of urgent care and regular family practice, all rolled in to one. We bring our patients personal and professional medical care that you would expect from a primary care doctor, plus advanced tools for speedy treatment with same-day and walk-in appointments

On this web portal you may securely:

  • Confirm your appointment
  • Pre-Register for initial visit
  • Enter or modify personal information
  • View your medical history, medications and Lab Results
  • Communicate with our staff
  • Pay your bill and receive online patient statements (COMING SOON)

ESTABLISHED PATIENTS
If you are currently a patient who has provided a current email address, you can use the secure portal for functions listed above and many more.  For medical emergencies, please call us directly.

NEW PATIENTS
To become a new patient, please call our office to schedule an appointment after which time we will email you additional information on how to complete the necessary paperwork online. You must schedule an appointment BEFORE completing online paperwork.
